Required Documentation for Background Investigations

The following is a list of documents that will be required to progress through the application process upon successful completion of the written exam and physical agility testing.


  • Your birth certificate. U.S. Citizenship is required for a sworn position (Police Officer).
  • Your Social Security Card.
  • Your driver’s license.
  • Any college degree and transcript, as well as your high school diploma and transcript or General Education Diploma (GED)
  • If you have served in the U.S. Military for any period of time, your DD-214 (Member 4 Copy listing Characterization of Service, Separation and Re-Enlistment Codes) or a copy of your discharge certificate.
  • Any licenses or certificate which show special qualifications or skills. If you are a certified law enforcement officer, your state certificate and training academy curriculum with hours.
  • Signed and notarized Authorization for Release of Information.
  • Signed and notarized waiver for Release of Credit History Report.
  • Typed or Clearly legible, thoroughly completed Personal History Statement. This is a lengthy document and it is recommended that you download it to your personal computer prior to completion, as it is not possible to save your progress during completion otherwise.
